To work in a consistent and structured manner with disabled adults, and their carers to enable our workers to assess, determine and deliver their care needs.

How to Access the Services

Visit or telephone any one of our 13 Customer Service Centres where a contact form will be completed and forwarded to the appropriate adult or children's services care management team. The Care Management team will then assess your eligibility for the services provided by the Council. If you are eligible for any services a Care Plan will be compiled and a Care Co-ordinator will determine how the services are to be delivered. Please note that you may be required to pay for some services within your Care Plan.

Children and Families

• We help families which are having difficulties, and protect children at risk from abuse.
• We advise about rights and benefits.
• We prepare young people in care for independent living and support young people who have left the care system.
• We monitor the quality of services for children aged under eight.

We also provide:

• Multi - agency family support services (MAFSS) for children in need and their families.
• Information for people who young children needs services such as nurseries or childminding.
• Support for families so that, wherever possible, children can stay in their own homes, rather than go into care.
• Care in residential homes, with foster carers or in adoption placements, for children who cannot live at home.
• A service for people who want to adopt or foster children.
• An independent children's rights service.

Older People

• Advice and practical assistance to help people remain in their own homes and live independent lives.
• Intensive home care services for people who need help with personal care and practical tasks in the home.
• Home care and meals on wheels.
• Aids and adaptations such as bath hoists and raised toilet seats in the home of frail people and people with disabilities.
• Day centres where people can meet and take part in activities.
• Residential care both long term and short term for people assessed as no longer being able to live independently at home.
• Respite care for short breaks
• Direct payments so those who want to can organise and manage their own care
• Support to informal carers

People with Physical Disabilities

• Community based facilities enabling people with physical disabilities to live in, or close to, their own homes, including short breaks and direct payments
• Occupational therapy to people who need help in rehabilitation or in maintenance.
• Practical help and advice for those who look after people with physical disabilities.
• Support for people with sight or hearing problems, and arrange for aids and adaptations in people's own homes.

People with Learning Disabilities

• We provide residential homes. Some people live here for long periods, others come for short stays to give the people who usually care for them a break.
• We visit people at home, to give advice and practical help.
• We run day centres to help people with learning disabilities to develop new skills and give the people who care for them a break.

People with Mental Health Issues

We provide:

• A range of different services such as support groups, supported lodgings and day centres. These enable people to find the kind of care appropriate for their needs, working in partnership with Health colleagues as an integerated service.
• Support for families of people with mental health problems.
• We work closely with health authorities and voluntary organisations to help people to live as independently as possible.

People who Care for Others

The East Riding of Yorkshire Council aims to improve support to carers who are recognised as providing a substantial amount of care within the community.

The Council has therefore adopted a Carers' Charter:

• Carers should be recognised as individuals with their own expertise, skills and needs
• Carers should be able to make informed choices about taking on/ continuing the role of carer
• People should not make assumptions about a carers ability to provide care on the basis of race, culture, age, disability or sexual orientation
• Carers should be regularly consulted and have the opportunity to be fully involved, individually and collectively at all levels
• Carers should have available information that is accurate, honest, comprehensive and accessible
• Carers should have available a range of appropriate and flexible services to be available to carers, which meets individual needs
• Carers should be informed about available benefits
• All agencies should work together to provide efficient and affective services for carers

The Council also offers Carers Assessments, services for carers to enable them to carry on in their caring role, and a Carer’s Advisory Service to provide support, information and advice.
