If you:
- Value highly your independence in the community
- Are receiving support from mental health services
- Are willing to work with our project workers
- Feel you need a little extra help with your confidence in managing , either on your own or in a relationship
- Sometimes feel isolated
Then Home-link could be the right service for you. Please refer to the criteria below to see how homelink could help you stay in your home:
- You must be living in rented accommodation (private or local authority) and have a tenancy agreement.
- Are currently in hospital, receiving treatment and support for your mental heatlh needs and will need long term support and accommodation following discharge. Or, are already living independently and receiving mental health services, but finding it difficult and require ongoing support.
- Have drifted from one short term living arrangement to another and would benefit from additional support to help you find and maintain a stable, long term tenancy agreement.
- Have acknowledged long term mental health needs i.e. history of hospital admissions and/or receiving community based mental health services.
- Are vulnerable.
- Are able to cater for your own basic living needs with minimum support.
- Are willing to work with project staff in accordance with the aims of this scheme.
- Are able to demonstrate a local connection or the benefits of living in the area.
Unfortunately, this project can not assist those who :
- require short term or acute admission
- have primary problems of drug or alcohol abuse and require specialist care for those
- have a persistent history of causing physical harm to others
- regularly exhibit uncontrolled abusive or aggressive behaviour
- have a serious and persistent history of self harm
- are under the age of 18

How to access the services available
If you would like to discuss these or any of the services mentioned in more detail please discuss this with your mental health support worker or visit anyone of our 13 Customer Service Centres where a referral form can be completed and forwarded to the Social Service's department. The Care Management team will then assess your eligibility for services provided by the Council. If you are eligible for any services an assessment is undertaken. Based on your needs a Care Plan will be compiled and a Care Co-ordinator will determine how the services are to be implemented/provided.
