The Physical Disabilities and Sensory impairment team work with your care co-ordinator and other team, for example, occupational therapy to offer a range of services that will meet your needs. Services include:

Residential Care
Residential Care - The Physical Disabilities and Sensory Impairment Team value the importance of independent living and thus try to provide most services within the home or through day care. However, Applegarth Residential and Physical Disability Unit provides short term residential rehabilitation and respite care to help people who are coming out of hospital, or to prevent people having to go into hospital.
How to access the services available
If you would like to discuss these or any of the services mentioned in more detail please visit anyone of our 13 Customer Service Centres where a referral form can be completed and forwarded to the Social Service's department. The Care Management team will then assess your eligibility for services provided by the Council. If you are eligible for any services an assessment is undertaken. Based on your needs a Care Plan will be compiled and a Care Co-ordinator will determine how the services are to be implemented/provided.
