How to access the services available
If you would like to discuss these or any of the services mentioned in more detail please visit anyone of our 13 Customer Service Centres where a referral form can be completed and forwarded to the Social Service's department. The Care Management team will then assess your eligibility for services provided by the Council. If you are eligible for any services an assessment is undertaken. Based on your needs a Care Plan will be compiled and a Care Co-ordinator will determine how the services are to be implemented/provided.
Further information can also be found by using the Contacts and Links in the right hand navigation bar.
Please note that you may be required to pay for some services within your Care Plan. Visit Service Charges for more details.
