    Introduction

If you need end of life care, you may think that you have to move away from home. However, a lot of care services can be provided to you in your home by the community palliative team, or as a day patient at a hospice or hospital.

If you are in hospital, your care will be provided by the hospital doctors and nurses, as well as the hospital palliative care team. Depending on your needs, other health and social care professionals may be involved in your end of life care, such as:

    • your GP
    • community nurses
    • counsellors
    • our social care team
    • chaplains (of all faiths or none)
    • physiotherapists
    • occupational therapists
    • complementary therapists.
