Moving to our adult social care service may feel like a big step, but don’t worry our teams are here to help make it as easy as possible for you. There are other organisations that can help too.

Futures +

Futures+, are a social care team at the council who support young people and adults with a range of disabilities, including:

• a learning disability
• autism spectrum disorder
• physical disabilities
• sensory impairments.

Some of the key roles that the team may be able to offer you as an individual are:

• helping individuals to prepare for their future adulthood
• independent travel training
• travel pass assessments
• road safety assessments and skills training
• life skills observations
• mobile phone training (keeping safe)
• helping you know what’s going on in your local area and community.

Helping you get a job

A 'supported internship' helps young people aged 16-24 with a statement of SEN, or an Education, Health and Care plan. We do this by teaching you the right skills you need through learning in the workplace.

Community wellbeing teams

Our community wellbeing teams are responsible for assessing and supporting people aged 18 or over who may require care and support because of an illness or a physical or mental impairment.
