Moving to adult social care

Your care and support plan

On this page:

    Introduction

Moving from childhood to adulthood is a very important stage of your life as you will need to make plans for your future care and support needs to enable you to live as independently as possible. A care and support plan will help you to consider all of the options and decide on the best way forward.

    Creating your support plan

Reviewing your care and support plan

We understand that you are going through a period in your life where things are changing a lot, this might be because you:

    • have developed new interests
    • are leaving school or education
    • would like to go to college
    • want to move away from home
    • have new or different needs to those we previously assessed.

It’s important that your care and support plan is kept up to date so you are properly supported.

    Mental Capacity Act

If we feel you need help to make decisions about your care and support needs, we’ll arrange a mental capacity assessment.

    1. Your appointment

If your adult supported assessment has been completed and it has confirmed that you have eligible needs, we’ll arrange an appointment to help you create your care and support plan. We’ll normally visit you at home, school or college.

    1. At your appointment

At your appointment, we’ll help you to put together a care and support plan of how we will meet your eligible care and support needs.

    This will include:

    • what your needs are
    • what’s important to you and the things you like doing
    • your usual routine and the tasks you need to carry out
    • which adult social care services you will be using to meet your needs
    • how your family and friends may be able to support you
    • any specialist help you may need.
    1. Review

Once we have agreed what your care and support package will look like, it is then discussed and approved by a senior social worker or team manager. Your support worker will then contact you to let you know the outcome and how you will access any services.

    Initial review

Around 6 weeks after your support begins, we’ll invite you for an initial review of your care and support plan. This is to make sure you are happy, and everything in the plan is going well.

    Yearly reviews

Around a year after your care and support begins, we’ll make an appointment with you to review your care and support plan.

You can also ask someone else to attend this review with you, such as a:

    • member of your family
    • representative from school or college
    • personal assistant who supports you
    • care provider.

This review is to make sure that your current care and support plan still meets your needs.

You will then have a review each year while we are providing your care.

What to do if something changes

If your needs change before your yearly review, or you feel that your care and support plan isn’t meeting your needs, you can request a review at any time.

To request a review, speak to your support worker.
