    Introduction

    Looking after your health and overall wellbeing is an important part of staying independent.

    There are several resources available to support you to improve your health, connect with your local area and keep well for longer.

    Social prescribing

    If you need help to enhance your overall wellbeing or tackle social, emotional or practical needs that are impacting your health, your healthcare professional may refer you to the social prescribing service.

    This is a free service that helps you to:

    • create a personal wellbeing plan
    • set achievable goals
    • overcome obstacles
    • receive ongoing support and encouragement.

    It serves as a personalised guide to local resources, such as connecting those dealing with a dementia diagnosis to support groups.

    Who offers this service

    The service is run by Health and Wellbeing, Advisor and Connector workers, who are based in every East Riding GP practice. They come from various backgrounds and understand the challenges of making healthy choices and managing health needs. They've received thorough training, and keep up to date on the latest health information.

    Health and Wellbeing, Advisor and Connector workers make sure you get the right support by connecting you with:

    • various services, such as counselling, welfare support, housing advice and leisure centre health programmes
    • community groups
    • support networks
    • social care options.

    How to access this service

    You must be referred to this service. Referrals are usually done by:

    • your GP
    • the adult social care team
    • the hospital team, if you are leaving hospital.

    British Red Cross Local Coordination Service

    You can get support to build your confidence and enhance your wellbeing through the local coordination service. Available to East Riding residents aged 18 or over, this is a partnership with the British Red Cross.

    A team of skilled and dedicated local coordinators are ready to assist you to live independently and confidently in your community. They’ll take the time to understand your unique situation, goals, and challenges.

    Local coordinators are available to help with various needs, such as:

    • addressing loneliness
    • managing finances
    • finding suitable housing
    • connecting with community groups.

    They can also organise extra support from health, social care, and community experts to make sure you receive the right help when you need it.

    How to access this service

    You would usually be referred to this service by:

    • your GP
    • the adult social care team
    • the hospital team - if you are leaving hospital.

    However, you can also refer yourself or a loved one to this service. To make a referral:

    Please note: the Red Cross service administrator is available Monday to Friday, 9am-5pm. They will pass your details on to the local coordinator for your area.

    You and your family can get dedicate support to take steps towards a healthier lifestyle from Healthier Futures.

    Healthier programmes

    Healthier Futures offer three free expert-led programmes to help you and your family to become healthier.

    When you sign up for a programme, you’ll be asked to take a free assessment to check that you qualify to take part.

    Healthier Lifestyles - Join a 12-week programme to help you lose weight and live a healthier lifestyle.

    Healthier together - Aimed at children and young people, this programme helps with weight management.

    Quite Smoking - Get support to quit smoking with this 12-week accredited programme.

    Other support available

    Healthier Futures offer personalised advice and guidance on various other aspects of health and wellbeing, including:

    • alcohol reduction
    • mental wellbeing
    • nutrition
    • physical activity
    • sleep.
