To help you through each stage of your child growing up, we’ve created this transition timeline.

It gives an overview of information and services related to special educational needs and disabilities (SEND) for people up to age 25 and their families. This includes education, health and social care.

Not everything will apply to every pupil or young person, and it doesn’t cover everything, but it should help you understand what support is available, the services and teams who can help, and how to get in touch.
